The DePelchin Center recognizes the importance of father involvement in a child’s life and has created special programs for men. Each year, millions of Americans are discharged from the hospital and readmitted within 30 days. These readmissions are often due to a lack of coordination between providers, inadequate follow-up care, and poor patient education.

The Agency for Healthcare Research and Quality reports that effective transitional care can lead to better health outcomes and reduce the likelihood of a repeat hospital visit. Dr. Jason Nichols from WellMed Medical Group stops in Houston to discuss it.
